報名須知 / Registration Terms:

• 一經報名，費用概不退款，名額可轉讓予親友（請提前通知主辦單位）。 All registrations are strictly non-refundable, but transferable with advance notice.

• 主辦單位保留變更或取消活動之最終權利。 The organizer reserves the right to amend or cancel the event.

Terms and Liability Waiver

By submitting this form, I, the parent/guardian, agree to the following:

1. Liability: I understand that the organizers and volunteers will take all safety precautions, but shall not be held liable for any personal injury or property damage during the activity.

2. Medical Emergency: In the event of a medical emergency, I authorize the staff to seek professional medical treatment if I cannot be reached.

3. Media Release: I grant permission for photos/videos of my child to be taken during the workshop for church cultural archive and promotional purposes. (Please inform staff upon arrival if you wish to opt-out).
